Аннотация:

Эта статья открывает серию публикаций, посвященных актуальной проблеме эндоваскулярного лечения миомы матки — эмболизации маточных артерий (ЭМА). Авторы обладающие в настоящее время самым значительным опытом ЭМА в России, на основании собственного опыта и данных литературы обсуждают наиболее актуальные вопросы, связанные с применением ЭМА в лечении миомы матки и другой акушерской и гинекологической патологии. Среди них вопросы определения показаний и противопоказаний к вмешательству, результатов ЭМА (в том числе и в комбинации с другими методами лечения), вопросы оптимизации методики и разработки технических приемов, позволяющих выполнить ЭМА практически в любой ситуации, а также вопросы, связанные с выбором эмболизирующих препаратов. Авторы также детально останавливаются на т.н. «мифах» об ЭМА — сложившихся негативных взглядах на некоторые аспекты вмешательства, основанные на устаревших и неточных данных. Авторы делают вывод о значительных перспективах эндоваскулярных методов при гинекологической и акушерской патологии.

Аннотация:

В статье приведены подробные методические детали и технические аспекты эмболизации маточных артерий, имеющие важное значение для успешного выполнения и обеспечения безопасности вмешательства. Приведено подробное описание ренгеноанатомии внутренних подвздошных артерий, различных вариантов кровоснабжения матки и яичников, типов анастомозирования между маточными и яичниковыми артериями. Описаны технические приемы, способствующие успешному выполнению эмболизации маточных артерий при наличии сложных анатомических вариантов и особенностей строения маточных артерий.
